Steps To Reproduce:
1. Buy about 20 different old windows XP machines. 2. Try running Audacity on them. If you are (un)lucky on one or more of them, all the toolbar buttons will be repainted with the 'pause' button icon on it.

Also affects 1.3.14. According to the reports, mousing over the buttons redraws them correctly (though it does not necessarily redraw the button you hovered over).  

According to the Forum report, Audacity was installed on 15 "identical" XP SP3 machines and two of them have this issue.

FWIW I looked at Widgets bugs for toolbar buttons but could not find anything relevant.

Another report made today in 2.0.5 "Asus M3A78-T with a built-in ATI Radeon HD 3300 graphics card,running the ATI Catalyst 09.1 driver."

Request close as WONTFIX as we no longer support XP, and I suspect it could anyway be a bug in the video driver in how it repaints.
